What Is Ectropion?

Ectropion is a condition in which the lower eyelid turns outward, away from the eye.
This exposes the inner eyelid surface, disrupts normal tear drainage and leaves the eye vulnerable to irritation, dryness and infection.

It most commonly affects older adults due to age-related laxity, but may also occur after trauma, surgery or chronic inflammation.

If left untreated, ectropion can lead to persistent redness, recurrent infections and long-term corneal damage.

Symptoms of Ectropion

Common symptoms include:

  • Watery eyes

  • Irritation or burning sensation

  • Redness and inflammation

  • Dryness due to exposure

  • Stringy discharge

  • Recurrent conjunctivitis

  • Visible outward turning of the eyelid

  • Crusting along the eyelid margin

Severe cases may cause corneal exposure, ulceration or blurred vision.

Causes and Risk Factors for Ectropion

Ectropion develops when the structures supporting the eyelid weaken, stretch or become damaged.

Main causes:

1. Age-Related Laxity (Most Common)

Tissues and muscles lose tone over time.

2. Facial Nerve Weakness

Bell’s palsy or other facial nerve disorders.

3. Scarring (Cicatricial Ectropion)

From trauma, burns or previous surgery.

4. Mechanical Causes

Large tumours or eyelid swelling pulling the lid down.

5. Congenital

Rarely present from birth.

6. Chronic Eye Rubbing or Inflammation

Especially in allergies or dry eye.

Diagnosis and Assessment

Assessment includes:

  • Examination of eyelid laxity

  • Snap-back and distraction tests

  • Evaluation of tear function

  • Inspection for scarring or trauma

  • Assessment of facial nerve function

  • Corneal surface check for exposure damage

  • Persistent watery or irritated eyes

  • Difficulty closing the eyelids fully
